What Is Urogynecology?

Urogynecology is a specialized field focused on female pelvic floor and urinary disorders. It bridges urology and gynecology to address conditions affecting the bladder, urethra, and pelvic organs. Urinary Incontinence

When an individual has lost control of their bladder, they’re more likely to experience urinary leakage. While this may feel embarrassing, you’re not alone. Millions of people across the country struggle with urinary incontinence. There can be a range of conditions that can cause urinary incontinence. Although having any type of medical condition can be frustrating, many urinary disorders are treatable.

Conditions that Cause Urinary Incontinence

If you’re dealing with urinary incontinence, it could be the result of a range of medical conditions. Check out the following section to learn more about those types of problems.

  • Weak Pelvic Floor Muscles: These are the muscles that support the bladder in holding or releasing urine. Over time, they can begin to weaken due to factors such as childbirth, pregnancy, aging, and obesity. Without proper care, weak pelvic floor muscles can lead to stress incontinence, where leakage occurs during activities that increase abdominal pressure, such as coughing, sneezing, laughing, or lifting heavy objects.
  • Nerve Problems: Nerves play a crucial role in controlling the bladder and signaling when it’s time to urinate. Any type of nerve damage or dysfunction could potentially cause the bladder to become overactive (going to the bathroom too often) or underactive (the bladder doesn’t expel all of the urine). Conditions such as diabetes, multiple sclerosis, or Parkinson’s disease can affect nerve function and contribute to urinary incontinence.
  • Overactive Bladder Muscles: Some individuals may experience involuntary contractions of the bladder muscles, leading to an urgent and frequent need to urinate (urge incontinence or overactive bladder). An overactive bladder could be caused by a variety of factors, including neurological conditions, bladder irritation, urinary tract infections, and certain medications.
  • Obstruction of the Urinary Tract: An obstruction in the urinary tract can lead to urinary incontinence. In men, an enlarged prostate gland can put pressure on the bladder and urethra. This can make it difficult to start urinating or cause a sensation to urinate more frequently. In women who experience pelvic organ prolapse, where pelvic organs such as the bladder, uterus, or rectum bulge into the vagina. This can lead to urinary incontinence issues.

Pelvic Organ Prolapse

Bladder, uterine, or vaginal prolapse occurs when weakened pelvic floor muscles and supportive tissues allow one or more pelvic organs to shift downward, leading to pressure, discomfort, urinary symptoms, or a visible bulge. Options may include conservative therapies such as pelvic floor strengthening, pessary use, or minimally invasive surgical repair. Overactive Bladder

Overactive bladder is a common condition characterized by a frequent and sudden urge to urinate, often accompanied by urgency, leakage, or nighttime urination. Management may include lifestyle modifications, medication, bladder training, or advanced therapies when appropriate. Postpartum Pelvic Floor Dysfunctions

Pregnancy and childbirth can bring about various changes to a person’s body, including the pelvic floor. After childbirth, a person can develop postpartum pelvic floor dysfunction. While the issues from this disorder can significantly impact the quality of life, the professionals at Urology Associates of Cape Cod offer a range of treatment options to provide relief and improve bladder control.

Types of Postpartum Pelvic Floor Dysfunctions:

There are many issues that can occur as a result of postpartum pelvic floor dysfunction. If you’re struggling with problems after childbirth, browse the following section to learn more.

  • Pelvic Organ Prolapse: This condition occurs when pelvic organs, such as the bladder, uterus, or rectum bulge into the vagina due to weakened pelvic floor muscles. This condition can cause discomfort and pressure, as well as issues with urinating and passing stool.
  • Urinary Issues: Urinary incontinence (inability to hold urine) and retention (trouble emptying all of the urine out of the bladder) are two issues that people may experience after childbirth. Oftentimes, they occur as a result of weakened pelvic floor muscles and nerve damage.
  • Pelvic Pain: After childbirth, many people experience pain throughout their pelvic region, lower abdomen, and lower back. There’s a range of factors that can cause pelvic pain such as vaginal delivery, prolonged/complicated labor, tailbone injury, diastasis recti, SI joint dysfunction, and pubic symphysis.
  • Dyspareunia (Pain During Intercourse): Having any type of pain during intercourse is very uncomfortable. More than 62% of people experience dyspareunia in the first three months after delivery. It can be caused by a variety of things including hormones, scar tissue restriction, and tense pelvic floor muscles.
  • Pelvic Floor Muscle Weakness: Pregnancy and childbirth can weaken the pelvic floor muscles, leading to issues such as urinary incontinence, bowel incontinence, and pelvic organ prolapse. Strengthening exercises and pelvic floor rehabilitation can help strengthen the muscles.

Urethral Diverticula

Urethral diverticula are pocket-like outpouchings that form along the urethra and can lead to symptoms such as recurrent urinary tract infections, pain, urinary leakage, or discomfort during urination or intimacy.
